PUBLIC HEALTH NURSE II

Summary:

Performs a variety of public health nursing duties in the community setting. Primarily involved in providing home visiting case management services to families with children from birth up to age 21 years who are enrolled in Public Health’s Maternal Child Health home visiting programs (Healthy Start, Babies First, and CaCoon). Public Health Nurse provides parenting education and support to families; child growth and development screens and assessments; and serves as a case manager to develop, implement and monitor care plans and make referrals as needed.

Description:

Essential duties and responsibilities:

a) Reports to and consults with the Nursing Services Manager as needed and per program protocol.

b) Provides intensive home visiting case management services to assigned clients.

c) Provides a comprehensive assessment and periodic reassessment of child/family needs; case planning, implementation of the plan of care, coordination with other services providers, on-going monitoring the plan of care, referrals and linking to available/needed resources, and assisting the family to establish personal/family goals.

d) Provides home visiting services, per program policies/protocols, to increase family functioning, promote parent-child interactions, promote child growth and development, identify and set goals, promote problem solving skills, enhance self-sufficiency, reduce family stressors and barriers, and increase family supports.

e) Coordinates services when multiple agencies are involved with child/family to enhance child and family functioning and service delivery.

f) Provides on-going oversight of PHAs with whom the RN has delegated components of the plan of care including case conferencing and guidance. Assures the plan of care is being followed, remains appropriate, etc.

g) Provides input into development of annual plans and reports, as requested and per program policy.

h) Promotes Public Health home visiting programs to community partners and agencies. Provides training/orientation to partnering agencies on services provided and the referral process.

i) Provides/promotes screening of potential clients for eligibility and program placement.

j) Attends, and successfully completes, required training for the home visiting position. Participates in trainings as required. Maintains up-to-date skills and knowledge base related to early childhood and parenting issues.

k) Implements program objectives, and follows policies and procedures of the Maternal Child Health home visiting programs.

l) Collects and provides data for reports; assists with County, State, Federal, and grant reporting requirements.

m) Maintains client records per protocol. Completes clear, organized, accurate, and timely charting and billing.

n) Responds to subpoenas specific to clients served; prepares for and renders testimony as required.

o) Maintains all applicable professional, legal, and ethical standards including confidentiality and informed consent.

p) Learns/utilizes a variety of computer medical record systems, including use of newer office technologies.

q) Other duties as assigned.

 

Minor duties of position:

a) Participates in staff meetings, community meetings, and outreach activities as assigned.

b) Represents and collaborates with other Public Health and other early childhood programs in a positive manner.

c) Participates in maintenance of parenting resource room.

d) Participates in Medicaid Administrative Claiming time studies as assigned.

e) Participates in public health preparedness events and trainings as assigned.

Working conditions of position:

a) Usual schedule: Monday – Friday, 40 hours per week, 8:00 a.m. – 5:00 p.m.

b) Occasional evening meetings and/or evening or week-end home visits

c) Occasional out of town workshops/training, with possible overnight stays

d) Sitting at a computer, using keyboarding skills, for a variety of programs

e) Walking, bending, stooping, and climbing stairs to access client homes

f) Transporting equipment to/from home visits including lifting up to 20 pounds

Requirements:

Required education, skills, licenses, certificates, abilities, etc.

a) Candidate must meet Oregon Administrative Rule 410-138-0060 provider requirements and provide Targeted Case Management allowable activities/services as indicated in OAR 410-138-0007. Prefer candidates with at least two years experience working with families with young children. Prefer candidates who are familiar with local resources and have experience working with local service providers and systems. Prefer candidates who can speak, read, and write Spanish.

b) Current RN license required.

c) Bachelor’s degree and experience in nursing, human services, health occupations, or health education fields; a minimum of two years experience in the field of Public Health, Mental Health, or alcohol/drug treatment settings; or any combination of experience/training demonstrating the ability to perform case management is required. Prefer candidates with experience/knowledge related to home visiting, early childhood development, psychology, sociology, or teaching fields. Knowledge of principles and methods relevant to home visiting a plus.

d) Successful completion and subsequent certification with Parents As Teachers and KIPS program and/or subsequent adopted curriculum required. Motivational interviewing skills, and infant massage and/or lactation certification a plus.

e) Must have knowledge of infant and child development, parent-child attachment, dynamics of child abuse/neglect, dynamics of domestic violence, and community resources, and be able to monitor and assist PHAs to follow case plans and family goal plans that relate to identified strengths and needs of individual families.

f) Must be able to establish positive working relationships with culturally diverse families experiencing issues such as domestic violence, child abuse/neglect, altered growth and development (neurodevelopment, developmental milestones, identifying actual or potential delays, etc), nutritional issues, poverty, etc. Must relate effectively to individuals of varied ages, ethnic, socio-economic, and cultural groups; possess the ability to exercise tact, discretion and judgment in working with a variety of people.

g) Must be able to assess, explain, demonstrate, and model good parenting behaviors and sound problem-solving techniques.

h) Must possess strong observational skills to assess, implement, and evaluate a plan of care based on the changing needs of a family and to enable accurate and thorough assessment of family strengths and vulnerabilities.

i) Must be able to remain calm and emotionally available in an environment serving at-risk families.

j) Must have the ability to communicate effectively, both orally and in writing, with co-workers, individuals, small groups, and large groups using good grammar, spelling, and punctuation.

k) Must possess the ability to establish and maintain positive, professional, and congenial working relationships with staff, clients, and community partners.

l) Must possess excellent time management skills, be able to prioritize tasks in a fast paced environment, be able to work efficiently and independently with little supervision to meet program expectations and timelines, and be flexible and adaptable to fluxuating priorities.

m) Must be willing to receive and act upon feedback.

n) Must be "computer literate" at a level sufficient to efficiently maintain case notes, monthly statistics, bill for services, generate and write reports, and communicate via e-mail.

o) Must have a valid Oregon Driver’s License (or ability to obtain one) with a good driving record.

p) Must successfully pass a criminal background check

Operation of a motor vehicle: Required

a) Home visiting materials such as the PAT curriculum; miscellaneous assessment and screening tools

b) Home visiting tools such as infant scale, length board, and eye occluder, etc.

c) Operation of an IBM compatible computer/laptop and related programs such as Microsoft Word, various databases, e-mail, and other programs

d) Cell phone, telephone, satellite phone, fax machine

e) Photocopier, TV, DVD and other related equipment

f) Digital camera or video recorder
