Responsible for delivering comprehensive nursing care as an advanced nurse practitioner in the following areas: complete a comprehensive medical assessment that may include a developmental assessment, a psycho-social assessment, and or an assessment of family functioning.
Obtain a health history and perform a physical exam.
Assess and provide health education to individuals and groups about health maintenance and management of acute and chronic conditions.
Provides anticipatory guidance related to family planning to meet the client’s needs.
Orders and/or performs appropriate diagnostic and screening tests and collects additional data for evaluation of illness/wellness.
Able to develop a differential diagnosis and risk identification through assessment and interpretation of data.
Demonstrates prioritization of problems and issues based on assessment data and the individual/family perceived priorities and needs.
Integrates knowledge of clients’ needs and medical interventions to design care plans and take actions that are consistent with the standards of practice, licensing laws, and established policies and procedures.
Provides managed patient care by developing and implementing individual treatment plan: prescribing/dispensing medications; providing appropriate referrals; ensuring education, counseling, and case management follow-up as necessary with other professional and community agencies.
Effectively uses electronic health information retrieval systems, data-collection processes, and technology applications for patient’s records, fiscal management, tracking, reporting, and maintaining information for individuals, populations and/or program needs.
Delegates and supervises tasks assigned to other staff members.
Participates on interdisciplinary teams to achieve designated client outcomes.
Works towards and promotes each Programs objective.
Completes documentation in a timely manner and per established protocols and policies and participates in chart reviews as necessary.
Works with management on quality assurance activities involving clinical practice that contribute to the overall effectiveness of the clinic. Assists with protocol development and revisions as necessary.
Identifies internal and external issues that may impact delivery of essential medical and public health services.
Assists with orientation of staff and students and acts as a resource person to all staff. Participates in student educational programs by orientating student to program area, selection of client cases for demonstration or teaching of advance practice nursing skill set, mentoring student, and providing evaluation to instructors and students.
Has knowledge about Public Health and Family Planning services and strives to remain knowledgeable about community needs and state and federally funded programs and goals.
Assumes responsibility for own professional development by pursuing education, participating in professional committees and work groups, and contributing to a work environment where continual improvements in practice are pursued.
Works collaboratively with others.
Oral and written communications must be clear and effective.
