Lead Specialist/Community Health Worker Abilene 36759BR Position Description Performs advanced, specialized work to assist in the organization and implementation of various projects and programs. Work is performed under minimal supervision within established policies with latitude for independent judgment. Works on special projects as assigned. May substitute for supervisor during his/her absence. Major/Essential Functions Maintain daily schedule of client visits and other work-related appointments Maintain all program and clinical records as assigned Be a referral resource for both clients and community organizations Maintain directory and updated contact info for community organizations Be a liaison for community organizations and clients Plan and attend community outreach activities Track assessment forms and reporting data, personal mileage, and all other project forms collaborate with clinical staff to assure quality control related to the project Meet recruitment and referral goals Provide project-specific education to community members and clients Observe privacy laws related to patient data at all times Navigate clients to clinical services in collaboration with the health clinics and hospitals Assist clinics and hospitals with meeting project objectives Maintain certification by attending required continuing education Assist development of training and educational materials Attend community events including health fairs, local celebrations, school events, and other events where the project's eligible population are available Required Qualifications High school plus six years experience in related field. Additional education may substitute for experience on a year for year basis.
