DUTIES AND RESPONSIBILITIES Primary Patient Care: In specialized or primary care areas of clinical expertise, primarily examines, diagnoses, and treats patients, including those referred by Nurse Practitioners and Physician Assistants. Diagnose illnesses and prescribe and administer medical treatment, including medication abortion services. Counsel students on related medical/personal problems and health measures and refers, when appropriate, to other health care professionals for diagnosis and treatment. Exhibit concern for both quality and cost of care; deliver quality patient-centered care with compassion; emphasize patient education when possible, including prevention and self-care; refer patients to Health Promotion for additional educational activities. Treat and follow up on treatment to ensure appropriate resolution of illness or injury; consult with private physicians and public agencies in securing treatment for students requiring services beyond those available on the campus. Maintain medical records with honesty and integrity; provide service in both private office and acute care setting; provide services during noon and evening hours of operation. Skilled in time management and exhibits flexibility to maximize patient service. Maintain broad privilege list in the credentialing process and exhibits a willingness to see a wide range of medical conditions; coordinate with campus Counseling Services and staff Psychiatrist for those patients presenting with mental health problems. Advise professional staff on developing health education and preventive medical care programs; participate in specialty clinics and staff conferences. As assigned and privileged, may perform minor surgery which does not require general anesthesia. Lead Physician Serve as a member of the Administrative Leadership Team; provide administrative leadership and completes administrative tasks for the clinical staff. Participates in the credentialing and privileging process for all clinicians. Facilitates and monitors professional peer review process; ensures that it meets AAAHC standards; provides the Health Records Technician the list of topics to be reviewed in the ensuing year. Serve as a consultant to the Director and contributes to performance evaluations of the clinical and ancillary staff. Directs, guides and determines schedule for an ongoing internal education process for clinicians, nurses and other health center staff. Serves as the Director of Laboratory and Radiology Supervisor unless delegated to another clinical staff member. Provides consultation regarding Pharmacy services. Advises the Director on clinical matters, policies affecting patient care and current practices. Represents the clinical staff in the development of written policies and procedures. Maintains liaison between the physicians, staff and the Director; serves special clinical functions or as a special consultant to SHS or university staff as needed. Consults with SHS physician staff on difficult clinical problems; reviews Incident Reports after evaluation by the Director if deemed necessary and is Chair of the Risk Management Committee. Serves on special committees as assigned; writes policies and procedures as needed especially on clinical matters. Reviews group physicals such as athletic physicals and international physicals and makes appropriate dispositions. Current medical board certification appropriate to assigned duties, e.g., pediatrics, gynecology, internal medicine, family practice. Possession of a current Drug Enforcement Agent license for prescribing physicians. Appropriate cardiopulmonary resuscitation certification. Completion of one or more approved residency programs in specialty appropriate to assigned duties; and two years or its equivalent of increasingly responsible experience in the practice of the specialty; prior or current affiliation with a hospital, ambulatory clinic or private practice is desirable. Thorough knowledge of the principles and methods of board certified medical specialty. Thorough knowledge of community resources in medical treatment and public health practices. Specialized knowledge of diagnosis of common medical disorders and injuries. Knowledge and familiarity with standards and ethics of the California Medical Association and American Medical Association. Ability to skillfully apply principles and techniques of current medical practices in the functional areas to which assigned; provide diagnostic treatment and medical counseling services in a campus medical program. Preferred Qualifications Knowledgeable in public health practices and procedures and ability to advise campus community in public health matters. Experienced in Adolescent Medicine. Expertise working in a CLIA waived laboratory setting. Certification for x-ray supervisor and operator. Proficient in the use of an electronic medical record system, specifically Point N Click. Job/Department Summary: Under the general direction of the Senior Director for the Student Learning Center (SLC), Cross Cultural Leadership Center (CCLC) and Student Transition and Retention (STAR) Center, and Affinity Centers, the Program Coordinator will serve as a member of the Asian American Native Hawaiian Pacific Islander (AANHPI) Center/CCLC staff, providing direction and advisement in educational and cultural programming for AANHPI student population. The AANHPI Center and CCLC provide a collaborative space to enhance academic and social integration for historically under-served student populations (i.e. Underrepresented Racial Minorities, First Gen, Pell-Eligible). Incumbent is also responsible for recognizing specific program needs and for collaborating with student affairs and academic departments to improve and develop approaches for implementation for student engagement and success. Kellogg Arabian Horse Center (AHC) is a legacy program of Cal Poly Pomona and promotes the historic W.K. Kellogg Arabians by providing a venue where students and the public can learn more about and engage with the Arabian Horse. The AHC provides an environment where students can learn about the Arabian horse in a learn-by-doing environment, supporting the educational mission of the university and continues the tradition of breeding and training of high-quality purebred Arabian horses. DUTIES AND RESPONSIBILITIES Horse Training Manage the development and training of each horse by yourself, or thru developing, directing, and guiding AHC staff who also work with the training and development of the center’s horses. Manage, develop, execute, and oversee the development of each horse’s training plans to ensure horses are physically and mentally prepared for success at the AHC and to ensure they are prepared for success after being sold at public auction. In collaboration with the Breeding Coordinator, oversee training and development of all horses under three years of age, ensuring each foal confidently wear halters, lead safely around the property, have confidence with trimming hooves by farrier, lead in front of mare, stand confidently for grooming and basic clipping. Ensuring self-confidence in each foal. Horses three and under should be confident with leading around property, loading and unloading from trailer, stand quietly for bathing and farrier, and when tied. Ensure that the horses are overall safe to handle. In collaboration with Coordinator of Horse Training staff, ensure horses in their “three-year-old” year are under saddle and meeting Arabian Horse industry targets. After horses three-year-old year, advance their individual training in alignment with Arabian horse industry best practices according to age. In collaboration with Student Activates Coordinator, ensure new horses are being developed for student riding lessons and Sunday Shows. Oversee advanced levels of horse training for horses that are presented in the monthly Sunday Show, continually elevating the entertainment level of the exhibition while ensuring the shows have well prepared, safe, sound, horses. In collaboration with AHC staff, ensure horses are safe for all teaching labs and other activities or experiences where horses are incorporated. Identify individual horses that may be experiencing confusion or lack of understanding in their training, diagnose and develop new training plans for the horses so they become more self-confident and have a better training experience. In collaboration with Executive Director, develop a set of standards and selection process for selecting horses to be retained for breeding, sale, or student use. In collaboration with the Executive Director, develop and update system to continually evaluate horses with an emphasis on determining which horses will be retained and which will be sold at public auction. Develop and submit related reporting to the Executive Director (i.e., horse progress reports, stallion breeding selections, etc.) Internal and External Operations Manage and oversee livestock technicians and Student Activities Coordinator, to accomplish goals including instructional support. This may include providing mentorship to further the professional development of staff who work with the center’s horses. Work effectively with a diverse group of staff, students, and student employees and interact with the public and Arabian Horse Industry in a professional and congenial manner. Manage horse center related operational projects, including operating tractors, skid-steer, UTV’s and truck and horse trailer and minor maintenance projects. Manage the movement of horses in pastures ensuring horses are moved on a timely basis. Manage and assist AHC staff with routine herd health care, including but not limited to administering vaccinations, de-worming, and other routine and non-routine herd health care procedures. Kellogg Arabian Horse Center (AHC) is a legacy program of Cal Poly Pomona and promotes the historic W.K. Kellogg Arabians by providing a venue where students and the public can learn more about and engage with the Arabian Horse. The AHC provides an environment where students can learn about the Arabian horse in a learn-by-doing environment, supporting the educational mission of the university and continues the tradition of breeding and training of high-quality purebred Arabian horses. DUTIES AND RESPONSIBILITIES Horse Training Manage the development and training of each horse by yourself, or thru developing, directing, and guiding AHC staff who also work with the training and development of the center’s horses. Manage, develop, execute, and oversee the development of each horse’s training plans to ensure horses are physically and mentally prepared for success at the AHC and to ensure they are prepared for success after being sold at public auction. In collaboration with the Breeding Coordinator, oversee training and development of all horses under three years of age, ensuring each foal confidently wear halters, lead safely around the property, have confidence with trimming hooves by farrier, lead in front of mare, stand confidently for grooming and basic clipping. Ensuring self-confidence in each foal. Horses three and under should be confident with leading around property, loading and unloading from trailer, stand quietly for bathing and farrier, and when tied. Ensure that the horses are overall safe to handle. In collaboration with Coordinator of Horse Training staff, ensure horses in their “three-year-old” year are under saddle and meeting Arabian Horse industry targets. After horses three-year-old year, advance their individual training in alignment with Arabian horse industry best practices according to age. In collaboration with Student Activates Coordinator, ensure new horses are being developed for student riding lessons and Sunday Shows. Oversee advanced levels of horse training for horses that are presented in the monthly Sunday Show, continually elevating the entertainment level of the exhibition while ensuring the shows have well prepared, safe, sound, horses. Identify individual horses that may be experiencing confusion or lack of understanding in their training, diagnose and develop new training plans for the horses so they become more self-confident and have a better training experience. In collaboration with the Executive Director, develop and update system to continually evaluate horses with an emphasis on determining which horses will be retained and which will be sold at public auction. Job Summary Under direct supervision of the Director of Student Diversity and Belonging, and with lead work direction from the MultiCultural Coordinator, this position provides support services under the Multicultural Center umbrella in SDAB with a focus on supporting Asian Pacific Islander Desi American (APIDA). Th APIDA Initiatives Coordinator assists in conducting outreach to APIDA affinity clubs and organizations to support their success at Cal Poly. DISTINGUISHING CHARACTERISTICS The Basic Needs division at Saddleback College serves student populations that experience unique challenges accessing nuanced support. These students require specialized support navigating federal, state, county, and local eligibility criteria to access resources that are oftentimes beyond the capacity of traditional community college programs and personnel. Help-seeking behaviors amongst these student populations can be limited due to the sensitivity of their identities. The ever-changing nature of law and policy can lend itself to unclear eligibility criteria which can negatively impact these students. Thus, access to these resources and services, or lack thereof, often shape the experiences of students and impact their academic trajectory, motivation, sense of self-efficacy and hope. The Case Manager will be familiar with special student populations such as foster youth, undocumented/Dreamer, LGBTQ+, formally incarcerated, parenting student, Latinx, first generation Black or African American student populations to contribute to the overall aim of supporting the holistic well-being of students to foster their success. Common student referrals to the Case Manager may include, but are not limited to: • Students experiencing food/housing/clothing/hygiene insecurities. • Students experiencing other personal and/or emotional challenges. • Students experiencing a major life change (traumatic or otherwise) that may be affecting their academic achievement. • Any referral submitted by faculty/staff for a student requiring non-mental health services. REPRESENTATIVE DUTIES • Coordinate and provide case-management services that are safe, timely, effective, efficient, equitable, and client-centered to individual students. • Conduct non-clinical assessment and plan that includes, but is not limited to, facilitate steps, remove barriers, determine eligibility, and navigate through complicated organizational processes. • Learn about and establish connections with off-campus and on-campus resources and agencies that provide services to targeted student populations. Maintain and update a centralized repository of on-campus and off-campus resources and services. • Manage and evaluate crises by helping the student make informed decisions and act as their advocate regarding their status, housing, academic, health, well-being, and treatment options. • Facilitate multiple care-aspect (case coordination, information sharing, follow-up, monitoring). • Take the extra mile and interact with the student to keep track of their progress and to ensure satisfaction. • Maintain accurate and timely case records (forms, referrals, and follow-up). • Record and maintain data collection and assessment related to the development of case management plans, outcomes, and statistical reports. • Design and implement interventions that will help students meet academic and personal goals. • Participate in relevant community outreach and on-campus committees. • Provide in-service training to staff on related topics. • Help students achieve wellness, service utilization, and increase autonomy. • Adhere to professional standards as outlined by protocols, rules, and regulations. Depending on program need, counselors are classified as Student Services Professional, Academic-Related AY or 12-Month. Appointments from the part-time pool are temporary, with the possibility of renewal. Minimum Qualifications: Demonstrated experience functioning at a high level as a generalist counselor in a diverse setting is required. This position requires a valid California license as a psychologist, social worker, marriage and family therapist, or licensed professional counselor (or eligibility for licensure in the state of California.) Strong clinical skills and knowledge of best practices in the field. Ability to work within a brief (3-6 session), non-weekly therapy framework. Strong interpersonal skills; ability to effectively interact with a diverse client population; ability to establish and maintain long-term relationships with non-student leadership of campus departments and off-campus organizations.
