What You'll Be Doing We are currently seeking a Housing - Social Worker to become part of the Housing Affordability and Community Revitalization (HACR) team! Housing - Social Worker works with Permanent Supportive Housing programs and reports to the LCSW- Supervisory within the Cornerstone Program. This interdisciplinary team provides case management, support, counseling and guidance to special needs populations. This position is ideal for a passionate Housing - Social Worker who is ready to make a direct impact in the lives of program participants with special needs in our community. Responsibilities include: Assessing the needs of program participants and develops individualized case plans to lead program participants to greater self-sufficiency and retention of permanent housing Facilitating relationship building and connections to Community resources, services and partners Conducting home visits at a minimum of once per month and more frequently as needed Providing appropriate resources, tools and counseling to assist residents in achieving their case plan goals Providing transportation as necessary Assisting with identifying suitable housing options based on the client’s needs Assisting program participants with obtaining important documents ( i.e., social security cards, birth certificates, driver’s license and award letters, etc.) Records case notes and related information in the Homeless Management Information System (HMIS) and/or others systems in a timely and accurate manner Position may perform other duties in addition to the above as assigned by management. Not only do more units of affordable housing need to be built, housing services must be provided to support families and individuals to access or maintain safe, decent, affordable housing. The Basics (Required Education and Experience) Bachelor's degree in Social Work or related field One year of job-related experience Valid Driver's License and "safe" driving record Equivalent education and experience are accepted Beyond the Basics (Preferred Education and Experience) Master's Degree Two (2) years experience working with either Chronic homelessness, mental health, substance abuse and/or people living with disabilities License/Certifications in Outreach and Community Engagement/CASAC Certification, Peer Support How Will We Know You're 'The One'?
