Summary
Join a growing clinic that is looking for a Family Medicine provider. You would be Responsible for the delivery of patient-centered, team-based care falling within the scope of practice. Works collaboratively with Nurse Practitioners, RNs, LVNs, Social Workers, Case Managers and Medical Assistants to provide physical and psychosocial assessments, diagnosis and management of chronic and acute health and illness needs. Assumes responsibility and accountability for the continuity of healthcare and health education regardless of the presence or absence of disease. Utilizes standardized procedures and consults the Medical Director in cases outside of scope of practice.Generous compensation, full benefits are offered including but not limited to Health/Dental/Vision, paid time off, 401K with employer matching along with a sign-on bonus!
