    Post Acute Care Physician opportunity in Media, PAGreat full-time (5 days per week) primary care role, you will have the opportunity to manage complex patient cases in post-acute settings (skilled nursing and independent living facilities) in Media, Pennsylvania, focusing on improving care for some of the most vulnerable patients.Position Highlights:Uncapped earning potentialPaid professional liability insurance with tail coverageLeadership and growth opportunities to further your careerAccess to professional development tools, educational resources and CME through in house InstituteAccess to our clinician wellness and referral programPrimary care rounding in skilled nursing facilities- Flexible rounding scheduleManage chronic disease, diagnose illnesses and medical conditions, and implement plan of careProvide oversight, guidance, and mentorship to our nurse practitionersConduct clinical case reviews, as needed and share tools, information, and guidelines as they relate to quality of careQualifications:Minimum of 1 year of experience, with a commitment to delivering high-quality, patient-centered carePost-acute care experience is beneficial, though not required, as we provide comprehensive support and development opportunitiesActive and unrestricted Pennsylvania license (or in process)Active and unrestricted DEA/prescriptive authority (or in process)Internal medicine (IM), family medicine (FM), emergency medicine (EM), hospitalist or candidates with any related experience are welcome to applyCompensation:Compensation is fee for service (FFS).
