    Job Summary: We are seeking a friendly and compassionate Pulmonary Nurse Practitioner/ Physician Assistant to join our growing team. In this role, you will provide comprehensive pulmonary care to patients in post-acute care settings while collaborating closely with physicians, nursing staff, and interdisciplinary care teams.     Responsibilities:

    • Conduct pulmonary assessments for patients in post-acute care settings
    • Evaluate, diagnose, and manage pulmonary conditions including COPD, asthma, pneumonia, and respiratory failure
    • Develop and implement individualized treatment plans
    • Collaborate with interdisciplinary teams
    • Educate patients, families, and facility staff on respiratory care and treatment plans
    • Monitor patient progress and adjust care plans as needed
    • Maintain accurate and timely EMR documentation
    • Support initiatives focused on reducing hospitalizations and Return-to-Hospital (RTH) events

    Requirements

    • Preferred a minimum of 6 months of experience in post-acute care setting
    • Preferred experience in Pulmonary: training provided
    • Active and unrestricted state Nurse Practitioner license or Active PA license and NCCPA certification required
    • Master s Degree in Nursing from an accredited Nurse Practitioner program or Master s Degree from an accredited Physician Assistant program required.
    • CPR BCLS/BLS certified
    • Strong communication, organizational, and teamwork skills
    • Ability to work independently in the clinical environment
