    Multidisciplinary Heart & Vascular Program is seeking a fellowship trained Advanced Heart Failure CardiologistMust be BC/BE in Cardiology and Advanced Heart Failure CardiologyContemporary experience with management of patients on mechanical circulatory supportFunctions appropriately as part of care delivery team through efficient use of resources and skills, both their own and those of other provider and staffActively participates in multi-disciplinary team approach (oversight of advanced practitioners)Responsibilities include continued development of the current Heart Failure Clinics to improve access and extend the scope of services to identify patients at risk for mortality, hospital readmission, and other adverse events to ensure appropriate evidence based medical and device therapies are providedFunction as a collaborative physician team member within the comprehensive inpatient Heart Failure service including consultative, diagnostic and treatment for high risk hospitalized patientsIdentification and evaluation of patients for LVAD candidacy including destination therapy and bridge to transplantation along with coordination of care with neighboring transplantation centers as appropriateFull complement Cardiovascular Service line (Electrophysiology, Structural heart with complex interventional cardiology, Advanced imaging, Cardio-Thoracic Surgery and Vascular Surgery)Involved in both outpatient and inpatient AHF responsibilities to include consultative with strong primary hospitalist service supportFull range heart failure program, including:Temporary mechanical circulatory support, including IABPs, Impella, RVADs, ECMO.Dedicated heart failure treatment unit with trained AHF nurses and excellent NP/PA supportRobust Durable LVAD program: HM2 and HM3, with myocardial recovery protocol in placeHeart transplantation (via our collaborating site)Cardio-MEMS remote monitoringHeartLogic remote monitoringHeart failure support and education program with dedicated nurse educators, a Disease management clinic and dedicated heart failure pharmacistCardiac MRI, PET, PYP scan and cCTA with CT FFR, one of a handful in the stateCardiac amyloid and Cardiomyopathy programsHypertrophic cardiomyopathy programPulmonary hypertension programCardio-genetics programCardio-obstetrics programHeart Failure prevention program Mitra-Clip and TAVRCardio-pulmonary exercise test (CPET)Right heart catheterization and endomyocardial biopsyGrowing portfolio of Heart Failure Clinical researchCardiac contractility modulation (CCM) Diuresis clinicAtrial and ventricular arrythmia ablation program Excellent Compensation and Benefits package including malpractice, Sign-on Bonus & Relocation availableH Visa OK
