Job Description

The Job

Arrowhead Regional Medical Center (ARMC) is recruiting for Community Health Workers (CHW)* - Contract for the Justice Involved Department to provide person-centered support to justice-involved individuals. Community Health Workers will work collaboratively with healthcare providers, correctional partners, and community-based organizations to coordinate medical, behavioral health, and social services that support successful transitions from incarceration to the community. Positions help members navigate healthcare systems, access community resources, and improve continuity of care and overall health outcomes.

Duties may include, but not limited to:

• Conduct outreach, engagement, and health education to encourage participation in healthcare and supportive services.
• Assess member needs and identify barriers to accessing medical, behavioral health, substance use, and social services.
• Coordinate and facilitate access to healthcare providers, behavioral health services, community resources, and social support programs.
• Support pre-release planning to ensure continuity of care following release from incarceration.
• Assist members in applying and navigating Medi-Cal benefits, healthcare systems, and available community resources.
• Collaborate with interdisciplinary care teams, correctional partners, and community organizations to coordinate services and develop person-centered care plans.
• Maintain timely, accurate, and compliant documentation in electronic health records and other required systems.
• Participate in interdisciplinary care team meetings to coordinate ongoing care.

This is a contract position which does not attain regular status as a County employee. Contract positions serve at the pleasure of the appointing authority.

Justice Involved (JI)

The Justice Involved (JI) program focusing on supporting individuals re-entering the community post-incarceration and reducing recidivism. Community Health Workers facilitate Medi-Cal enrollment, continuity of healthcare services, and general support services to detainees at four correctional facilities.

Enhanced Care Management (ECM)

The Enhanced Care Management (ECM) benefit is a foundational component of CalAIM. ECM is a whole-person, interdisciplinary approach to comprehensive care management that addresses the clinical and non-clinical needs of high-cost, high-need managed care members through systematic coordination of services that is community-based, interdisciplinary, high-touch, and person-centered.

CONDITIONS OF EMPLOYMENT



Background: Prior to appointment, applicants will be fingerprinted and must pass an extensive background investigation, including a polygraph examination. Applicants must not have been convicted of a felony.

Travel: Travel throughout the County may be required and employees will be required to make provisions for such transportation.

Basic Life Support Certification: Positions within Clinical Operations may require a Healthcare Provider level Basic Life Support (BLS) certification.

CHW Certification: Incumbents must successfully complete a Community Health Workers (CHW) Certification program resulting in a certificate within one (1) year of hire or be terminated.

Sponsorship: San Bernardino County is not able to consider candidates who will require Visa sponsorship at the time of application or in the future. Candidates must be able to present their legal right to work in the United States at the time of employmen t.

Minimum Requirements

Applicants mu st meet BOTH of the following EDUCATIONand EXPERIENCE recruitment :

EDUCATION
Possession of a high school diploma or equivalent is required.

-AND- EXPERIENCE: Applicants must meet one of the options listed below: Option 1: Certification as a Community Health Worker. A legible copy of the CHW certificate must be attached to the online application. Option 2:
One (1) year of full-time experience as a Community Health Worker, Promotora, or Health Navigator. Option 3: One (1) year of full-time experience in community outreach, customer service, a medical office, human services, Behavioral Health, or a Substance Use Disorder program setting. Qualifying experience must include performing needs assessments, conducting interviews, referring clients to services, or providing crisis intervention.

Desired Qualifications

Two (2) years of experience as a Community Health Worker, Promotora, Health Navigator, or in community outreach, customer service, a medical office, Behavioral Health, or a Substance Use Disorder program is highly desirable.

The ideal candidate will demonstrate cultural sensitivity and experience working with diverse and underserved populations facing barriers to wellness. Basic knowledge of medical terminology and proficiency with Microsoft Office Suite (Outlook, Word, Excel, and PowerPoint) is also desirable.

English/Spanish bilingual skills are highly desired. For consideration of bilingual assessments, applicants must indicate bilingual ability on the application. Employees may receive additional compensation for utilizing such skills when allocated to a bilingual position, as approved by the department. Bilingual testing may be required.
