Health Educator

Job Description

Overview

The Health Educator will plan, develop, implement, evaluate, and adapt evidence-based Health Education initiatives aligned with departmental priorities and emerging community health needs. This position supports a broad range of public health programming.

Responsibilities

• Completes patient intake using approved paper or digital forms

• Performs patient health screenings including, but not limited to, blood pressure, body mass index, hemoglobin A1c, and total cholesterol

• Provides patient education based on results and refer to appropriate programs and organizations and complete follow-up with patients

• Maintains confidentiality and follow all HIPPA guidelines

• Attends outreach events including, but not limited to, health fairs, wellness expos, and conferences as representative of the Health Education program

• Engages with attendees to share Helah Education services and provide education materials and incentives to attendees

• Tracks and log items distributed and contact made

• Conducts thorough research on requested health topic to ensure accurate and evidence-based information is being used

• Maintains cultural and lingual competence in activities

• Requests timely review and approval of materials created

• Facilitates and conducts community presentations, activities, and classes

• Assess community needs and departmental priorities to recommend implementations and modifications to Health Education initiatives

• Supports program evaluation for effectiveness via instruments such as surveys and pre and post-test

• Researches and reports on health trends to stay up to date on current health trends and emerging needs using reputable sources

• Collects and shares findings with supervisor in a timely manner

• Inputs patient information and records into the electronic health records (EHR) system

• Supports the program's record retention efforts including, but not limited to, uploading, filing, labeling, and tracking of patient records

Position Type and Typical Hours of Work

• Non-Exempt - Full Time

• Flexibility to work evenings, weekends, and holidays is a schedule requirement

• In the event of an emergency, employees are required to work to provide for the safety and well-being of the public, including the delivery and restoration of vital services

Minimum Qualifications

• Bachelors Degree (BA/BS)

• Two (2) years of experience

Certifications, Licenses or other requirements

Required

• A valid driver's license is required; successful out-of-state candidates must be able to obtain a valid Texas driver's license within 90 days of hire

Preferred

• Community Health Worker (CHW) Certification preferred

Basis of Rating

A recruitment consultant will evaluate all applications against the posted qualifications. The city may also conduct additional skill assessment tests, in addition to the panel interview
