DescriptionJoin the Pine Haven family! We are looking for a compassionate and experienced Social Services Coordinator to support the emotional and social well-being of our residents and their families. This position plays an important role in helping residents and ensuring they receive the support, resources, and quality care they deserve.Essential duties and responsibilitiesAssist with psycho-social aspect of potential resident assessmentsProvide tours to potential residents and familiesCoordinate, and complete the admissions of new residentsResponsible for providing residents and families with the social and emotional aspects of transitioning to our facilityMeet with residents and families regarding emotional, social, or physical needsAssist staff with challenging resident or family situationsProvide emotional support as resident needs ariseConnect residents and families with community and government benefit programs and resourcesCoordinate, facilitate and participate in service plans, and complete follow up tasks from the care planningInvestigate, document, and follow-up on resident rights concerns, including lost itemsDevelop social services programs to provide higher level of care for residents, families, and staffCoordinate schedules for residents and outside providers who come on-site, such as podiatrist, audiologist, ophthalmologist/optometristWork closely with support department staff and managers, including providing direction as needed to staff and sharing concerns with managersServe on the Administrative On Call RotationProvide Quality Assurance structure, review of programs, and implement QA initiatives as they are developedFollow infection control and universal precautions policies and procedures to ensure that a sanitary environment is always maintainedAttend and participate in required training, conducted at time of hire and/or annually, that will include training designed to assure resident safety: i.e. fire safety, missing resident, evacuations, disaster response, etc.Report any observed changes in a resident's needs and/or condition immediately to a license nurse or manager as appropriateRequirementsPrior social services experience in a senior living setting requiredAbility to make independent decisions when circumstances warrant such action.Ability to deal professionally with personnel, residents, family members, visitors, government agencies/personnel and the general public.Patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever level they are currently functioningAbility to seek out new methods and principles and be willing to incorporate them into existing social services programsMaintain good attendance, including timely arrivalSupport positive working relationships with fellow team membersInterreact, communicate, and collaborate effectively with othersAttention to detail, time management, and organizational skills requiredMust comply with Pine Haven's policiesUnderstands, supports, and is committed to Pine Haven's Mission, Vision and Values#J-18808-Ljbffr
