    The Stanislaus County Department of Aging and Veterans Services is seeking a dedicated and compassionate individual for the part-time Staff Nurse II role within our Multipurpose Senior Services Program (MSSP). The ideal candidate will possess a strong foundation in professional nursing care, combined with exceptional interpersonal skills and a genuine commitment to enhancing the quality of life for our older adult participants with significant medical needs.

    This candidate will not only bring a solid clinical background and nursing expertise but also demonstrate a keen understanding of care management practices. They will work collaboratively with our Social Worker Care Managers, understanding that teamwork is essential to effectively meet the diverse needs of participants. Effective communication, empathy, and a client-centered focus are crucial attributes for this role, as the candidate will engage closely with participants and their families to foster trust and ensure a holistic approach to care.

THE POSITION The Stanislaus County Department of Aging and Veterans Services is seeking a part-time extra help Staff Nurse II to work in our Multipurpose Senior Services Program (MSSP). The ideal candidate provides professional nursing care and care management for participants in their homes. The nurse and Social Worker Care Manager make up the MSSP team. In order to provide services to the medically frail adults of Stanislaus County, coordination and collaboration within this team is vital. By employing a collaborative approach, this team will conduct a comprehensive health and psychosocial assessment for each participant. This is an intermediate level position that provides care under general supervision.

    Typical Tasks

    • Observe and assess the patient's condition;
    • Certify and complete the Level of Care Certification which supports eligibility for MSSP services for all new and active MSSP clients on an annual basis;
    • Report suspected cases of abuse and neglect of children and adults to proper authorities, according to procedures and protocols;
    • Ensure participant record documentation meets program requirements;
    • Collaborate and consult with the Social Worker Care Managers in the development of the client's individualized care plan;
    • Assist in implementation of a variety of services detailed in the care plan for each client;
    • Collaborate with physicians and other health professionals regarding client needs;
    • Monitor participant's needs and provisions in the care plan;
    • Identify and develop support systems for the participant;
    • Conduct face-to-face assessments and/or annual reassessments with MSSP clients;
    • May consult with therapeutic multi-disciplinary team in reviewing patient progress and success of treatment and case management plans;
    • Complete Health Assessments for all MSSP intakes for enrollment;
    • Ensuring that prior to purchasing and authorizing Waiver Services, all other resources have been exhausted;
    • Participate on committees; attend meetings, in-service programs and workshops relevant to the area of assigned patient care or specialty;
    • Complete Opposite Discipline Annual Level of Care Re-Certifications;
    • Assist with oversight of RN/PHN interns on an ongoing basis as well as with visits to the client's home to review services;
    • Complete face-to-face home visits and phone contact with clients on a regular schedule and as needed; and
    • Provide operational oversight when necessary.


    Minimum Qualifications (Knowledge, Skills, Abilities, Education/Experience)

    KNOWLEDGE

    • Principles and practices of the nursing process;
    • The Health Insurance Portability and Accountability Act of 1996 (HIPAA) Privacy, Security and Breach Notification Rules;
    • Nursing procedures, techniques, equipment and supplies;
    • Principles of contact investigation and infection control;
    • Collaborative health planning with multi-disciplinary teams;
    • Medical terminology, modern nursing principles, methods and procedures; and
    • Use and effects of medicine, controlled substances, clinic supplies, equipment, and disease processes including communicable diseases, symptoms and treatments.
    SKILLS/ABILITIES
    • Perform in stressful and crisis situations, interpret policies and procedures, and apply them with good judgment;
    • Communicate effectively orally and in writing;
    • Administer treatments and medications to patients under the orders of a health care provider and/or the Nursing Practice Act;
    • Ability to establish and maintain effective working relationships with staff, clients, and other public agencies or personnel;
    • Use of various computer programs such as Windows and an Electronic Medical Record;
    • Follow complex oral and written directions and prepare clear and concise written reports, including documentation in the medical records;
    • Provide safe, effective and efficient nursing care;
    • Supervise, train and guide the work of Medical Assistants and other licensed and unlicensed personnel when requested; and
    • Leadership skills and oversee operational processes.


    EXPERIENCE/EDUCATION
    • Associate or higher degree in Nursing

    LICENSE/CERTIFICATION
    • Current license to practice as a Registered Nurse in the State of California; AND
    • After receiving Registered Nurse license, one (1) year full-time experience in a hospital/outpatient care setting or experience which utilized the development of critical thinking skills; AND
    • Current valid BLS/CPR certification.

    DESIRABLE QUALIFICATIONS

    • One (1) year working with older adults with significant medical needs.
