DescriptionThe Program Coordinator plays a key role in connecting systems, providers, and families across Alameda County. This role leads outreach and partnership efforts that strengthen referral pathways between providers, Managed Care Plans, and Help Me Grow Alameda County-ensuring young children, especially those with complex needs, get connected to the care and resources they need, when they need them. - Design and implement strategies to build and maintain partnerships with pediatric providers, Managed Care Plans, community organizations, and other child-serving partners.
- Conduct outreach to pediatric clinics and other sites to strengthen referral partnerships with Help Me Grow.
- Establish and strengthen referral pathways between providers, MCPs, and Help Me Grow to ensure smooth, timely connections to services.
- Support electronic screening and referral systems use (e.g., EPIC, OCHIN, FindHelp).
- Drive targeted outreach to enroll children with complex needs into appropriate care coordination services.
- Identify barriers to referrals and services and help improve communication and coordination across systems.
- Track outreach efforts, referrals, and partnerships to monitor progress and impact.
- Analyze and share data to inform outreach strategies and system improvements.
- Represent Help Me Grow at community events and meetings across Alameda County (including some evenings and weekends).
- Support data collection and reporting on screening and referral outcomes.
- Assist with evaluating pilot projects and summarizing key findings.
- Create partner communications (e.g., emails, newsletters) to share updates, opportunities, and resources.
- Collaborate with staff and partners to coordinate outreach, training, and support efforts.
- Participate in meetings, present program information, and recommend improvements to processes and strategies.
- Stay up to date on program requirements, regulations, and best practices.
- Use new tools and technology to work efficiently and effectively.
QualificationsMinimum Qualifications:Any combination of training and experience that would provide the required knowledge, skills, and abilities is qualifying. Additional experience and/or education can be substituted to meet typical qualifications. A typical way to obtain the required qualifications would be:
Three (3) years experience in quality improvement, outreach programs and/or related pediatric or early childhood roles and an associate degree* (or equivalent coursework) in social work, public health, social services, or a related field (*or equivalent professional experience - a degree/college coursework is
not required).
Travel within Alameda County to various pediatric practices is required to carry out job duties. Some travel outside Alameda County is occasionally required. Some evening and weekend work required. Meet all health clearances required for healthcare settings, as needed.
Knowledge of: - Healthcare systems, healthcare quality improvement, care delivery practices
- The community insured by Medi-Cal/Medicaid in Alameda County and the challenges and opportunities for the healthcare community in serving them.
- Child development, developmental screening, early identification and/or intervention
- Applicable federal and state laws, rules and regulations related to Medi-Cal/Medicaid, Enhanced Care Management, and CalAIM.
- Early childhood and early intervention systems and services; prefer knowledge of Alameda County services
- Experience working with or alongside electronic health records (EHRs) or referral platforms (e.g., EPIC, OCHIN, FindHelp, RedCap, DMS systems).
- Clinical and administrative workflows in healthcare or community-based settings
- Support technology change management in provider settings.
- Project management, contract management, and quality improvement methods
- Family engagement, multilingual populations, and anti-bias/anti-racist practices
- Community demographics and social determinants of health
- Data collection, reporting, and program evaluation practices
Ability to: - Build strong partnerships with healthcare providers, community organizations, and stakeholders
- Provide coaching and support to improve quality and program performance
- Analyze programs, workflows, and data to recommend improvements
- Plan, manage, and evaluate projects and service delivery
- Facilitate meetings, trainings, and presentations for diverse audiences
- Communicate clearly in writing and verbally, including technical information
- Manage multiple priorities, meet deadlines, and work independently
- Interpret and apply policies, regulations, and program guidelines
- Use data systems, software, and standard business tools effectively
- Deliver high-quality customer service and maintain professional relationships
Please note: - We are currently working a hybrid 2 day per week in-office schedule and employees also come to the office to attend meetings and as requested.
- All First 5 employees must live and work in California.
- The salary range for this position is $94,000 to $136,000 and as an independent government agency, the posted salary is the set salary range for this position. We cannot offer salaries above this posted range.
