It involves gathering information, determining eligibility, and guiding consumers through various programs. As an Aging Case Aide 2, you will perform the following duties:Referral Processing:Review and record incoming referrals received through telephone calls, mail, faxes, police reports, and other sourcesMicrosoft Software Skills:Create fliers, calendars, spreadsheets, and other materials for advertising and organizational purposesInformation Screening:Screen calls, identify presenting problems, provide information, and collect detailed referral dataEligibility Assistance:Determine benefits eligibility and assist consumers with completing applications for programs such as prescription coverage, tax rebates, housing, and transportationCase Documentation:Document all consumer contacts with thorough and accurate case histories, resolutions, and dispositionsCommunity Support:Participate in wellness fairs, coordinate holiday gift events, and offer guidance on agency services and community resourcesWork Schedule and Additional Information:Full-time employmentWork hours are 8:30 AM to 4:30 PM, Monday - Friday, with 60-minute lunch.Telework:You will not have the option to telework in this position.Salary:In some cases, the starting salary may be non-negotiable.You will receive further communication regarding this position via email.
