Serve as an advocate for the consumer, monitor the delivery of services, link individuals to services, develop social supports, and encourage self-determination.Full-time positions include excellent benefits! Bedford-Somerset DBHS is a family-friendly agency with paid time off, paid sick time, employee assistance program, and family medical leave.DESCRIPTION OF WORKAs a County Caseworker 2, you will perform the following duties:Service Monitoring: Responsible for monitoring, linking, and evaluating services. Link to other services and make referrals as neededDirect Contact: Provide regular direct individual contact and provide face‑to‑face case management services while meeting productivity standards as set for the programPlan Development: Schedule and facilitate meetings to develop an individualized service plan with involvement of the individual and/or familyInformation Management: Enter necessary information and maintain information in computerized database, state computer systems, and in charts in accordance with state, federal and licensing regulations and Agency policy. Obtain & update information within required time framesCrisis Support: Offer crisis intervention services and coordinate assistance as needed.Collaboration: Work with Agency staff, providers, and community resources to support individualsTransportation Support: Link individuals to transportation services or transport individuals to appointments as neededIn addition, you will attend meetings, staffings, and trainings as needed. You may provide coverage in Somerset County. You will perform other related duties as assigned.Work Schedule and Additional InformationFull-time employment.Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.On occasion, staff may need to work hours outside of the set work week to complete projects, meet work demands or in the event of an emergency.Hours may vary to meet the needs of the consumers.This position reports to the DBHS Office in Bedford for work.At times, may provide coverage in Somerset County.Telework: Upon hire, new staff begin training and are on probation for 6 months. New staff may be eligible for a hybrid of on‑site and remote work after 3 months of employment.You will receive further communication regarding this position via email. Check your email, including spam/junk folders, for these notices.Required Experience, Training & EligibilityQUALIFICATIONSSix months of experience as a County Caseworker 1; orSuccessful completion of the County Social Casework Intern program; orA bachelor's degree with a social welfare major; orA bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social service agency; orAny equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.Other RequirementsPA residency requirement is currently waived for this title.You must be able to perform essential job functions.Legal RequirementsYou must pass a background investigation.This position falls under the provisions of the Child Protective Services Law.Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including, but not limited to, PA State Police, Child Abuse history clearance, and FBI clearance.This position falls under the provisions of the Older Adults Protective Services Act.Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.VeteransPennsylvania law (51 Pa.
