Clinical Court Navigator
Support Individuals Who Are Justice-Involved in the Schenectady Community
The Clinical Court Navigator is responsible for connecting justice involved individuals with mental health needs to appropriate services and resources to promote successful diversion, engagement in treatment, and improved outcomes. Provide ongoing supervision, training, and support to the Peer Court Navigator and serves as a liaison among individuals, behavioral health providers, criminal justice stakeholders, and community resources. Specific duties include:
- Conduct initial screenings to identify individuals' behavioral health needs, determine appropriate referrals, and assess whether a more comprehensive evaluation is indicated.
- Connect individuals with appropriate mental health treatment services and community resources. Facilitate connections to Single Point of Access (SPOA), Forensic Assertive Community Treatment (FACT) teams, Intensive and Sustained Engagement Teams (INSET), case management services, Safe Options Support (SOS) teams, and other community-based services.
- Facilitate communication and collaboration among individuals, behavioral health providers, criminal justice stakeholders, state agencies, and other community partners regarding behavioral health service needs.
- Provide participants with basic information regarding the court process and explain commonly used court-related terminology in an understandable manner.
- Connect individuals with resources addressing basic and health-related social needs, including shelter, food, transportation, benefits, and other supportive services. Assist with or refer individuals for completion of applications for appropriate social service benefits.
- Provide training, education, and consultation to criminal justice stakeholders, behavioral health providers, and other community partners regarding navigator services, behavioral health resources, and available state and local services to strengthen referral pathways.
- Coordinate with the NYS OMH Diversion Center regarding program operations, system issues, service barriers, and opportunities to develop innovative solutions.
- Engage individuals in a respectful, person-centered, strength-based, recovery-oriented, and culturally competent manner that promotes inclusion, dignity, independence, and self-determination.
- Safely access to all areas including multiple levels accessible by stairs. Community-based services may include visits to individuals' apartments, including locations on second and third floors.
- Provide transportation to individuals when necessary, using agency vehicles. Employees must have access to and be willing to use a personal vehicle for authorized business purposes, including community meetings and meetings outside the office or program location.
