Director of Admissions – Skilled Nursing Facility | Brooklyn, NY
A large, well-established, family owned skilled nursing and rehabilitation center in Brooklyn is looking for an experienced Director of Admissions to join its leadership team.
This is a stable, well-run building. The Administrator has led the facility for many years, the Director of Nursing is steady and established, and ownership is supportive and genuinely open to new ideas. You'll work with a team that has been together for a long time and knows how to get things done.
The facility offers a full continuum of care, including short-term rehab, long-term care, on-site dialysis, and adult day health services. That gives you a lot to offer referral sources and families.
What you'll do:
- Lead the admissions process from referral through move-in, keeping it smooth and welcoming for patients and families
- Build and maintain relationships with hospital case managers, discharge planners, and other referral sources
- Review referrals for clinical and financial fit, and coordinate with nursing, social work, and business office on each admission
- Manage census tracking, payer verification, and admissions documentation
- Work closely with the Administrator and leadership team to keep census strong
Marketing is welcome but optional. If you enjoy getting out into the community and building referral relationships, there's room to grow into that. If you'd rather stay in the building and focus on admissions, that works too.
What we're looking for:
- Admissions experience in a skilled nursing facility, ideally in Brooklyn
- Familiarity with Brooklyn hospitals, referral sources, and the local admissions landscape
- Working knowledge of Medicare, Medicaid, managed care, and private pay
- Warm, organized, and responsive, with strong follow-through
