About the Role:

Quality Care 4U Community Services in Pittsburgh, PA is looking for a passionate and dedicated Program Specialist to join our team! In this role, you'll play a vital part in delivering meaningful support and services to individuals in our community, making a real difference every day.

Responsibilities:

Develop, implement, and monitor individualized program plans for clients and participants

Coordinate and deliver community-based services and support programs

Maintain accurate and up-to-date client records, documentation, and progress notes

Collaborate with interdisciplinary teams, families, and community partners to ensure quality care

Monitor program outcomes and report on progress toward established goals

Ensure compliance with regulatory standards, policies, and procedures

Provide advocacy and resource referrals to clients and their support networks

Collaborate with client and scheduling needs

Requirements:

Bachelor's degree in Social Work, Human Services, Psychology, or a related field

Prior experience in community services, case management, or a related role preferred

Strong knowledge of person-centered planning and community support services

Excellent written and verbal communication skills

Proficiency in documentation, recordkeeping, and case management software

Compassionate, client-focused demeanor with strong problem-solving abilities

Valid driver's license and reliable transportation preferred
