    Why Join Contra Costa Health?

Contra Costa Health is the largest county department in Contra Costa County with over 4,700 employees. Our mission is to care for and improve the health of all people in Contra Costa with special attention to those who are most vulnerable to health problems. We provide services through nine operational areas: Behavioral Health, Contra Costa Health Plan, Emergency Medical Services, Environmental Health, Hazardous Materials, Health, Housing & Homeless Services, Public Health, Contra Costa Regional Medical Center & Health Centers and Detention Health.

Contract Costa Health Plan (CCHP) Case Management team is looking to fill one (1) vacant Health Plan Outreach Representative (HPOR) position! This position will be located in Martinez.

Our Case Management team works closely with Medi-Cal, (future) Medicare, and Commercial members to assess their needs, coordinate care, and connect them with medical, behavioral health, and social support services. By addressing complex health and social needs, we help members achieve better outcomes and live healthier, more independent lives.

Through the CalAIM Department, which oversees Enhanced Care Management (ECM) and Community Supports, our team helps remove barriers to care such as transportation, housing, nutrition, and other social determinants of health. We collaborate with providers, community organizations, and other health plan teams to ensure members receive holistic, person-centered support that addresses both medical and non-medical needs.

In this role, you’ll have the opportunity to make a tangible impact by guiding members through the health care system, improving access to services, and helping them navigate complex programs. You’ll gain experience in innovative care delivery, work alongside passionate professionals, and contribute to programs that advance health equity across Contra Costa County. Every day, your work directly supports members in achieving better health outcomes and a higher quality of life.

We are looking for someone who:

    • Is passionate about helping CCHP members navigate the health care system and access the services they need.
    • Has strong communication and interpersonal skills and can build trust with diverse populations.
    • Is organized, detail-oriented, and able to manage multiple outreach and care coordination tasks.
    • Is able to work collaboratively with providers, community organizations, and internal teams to ensure members receive holistic, person-centered support.
    • Is motivated to support innovative programs like Enhanced Care Management (ECM) and Community Support under CalAim
    • Has experience or interest in case management, health education, or community outreach
    • Is committed to improving health equity and outcomes for members in Contra Costa County

What you will typically be responsible for:

    • Educating members about their benefits and Case Management/CalAIM programs.
    • Welcoming new members and connecting them with services and resources.
    • Helping members navigate medical, behavioral health, and social support services.
    • Assisting with member questions, concerns, and complaints in a professional and caring manner.
    • Making and receiving calls to schedule appointments and follow up with members.
    • Representing the Health Plan at community events, outreach activities, and provider meetings.
    • Keeping accurate records of member interactions and program participation.

A few reasons why you might love this job:

    • Make a real difference by helping members access health care and community resources.
    • Work on innovative Case Management and CalAIM programs like Enhanced Care Management and Community Supports.
    • Collaborate with a passionate, mission-driven team focused on improving health outcomes.
    • Engage directly with members and the community through outreach and education.
    • Gain valuable experience in case management, care coordination, and holistic care.

A few challenges you might face in this job:

    • Helping members navigate complex health care and social service systems.
    • Addressing multiple needs at once, including medical, behavioral health, and social supports.
    • Engaging members who may be difficult to reach or hesitant to participate in programs.
    • Managing competing priorities and keeping accurate records of outreach and follow-up activities.

    Competencies Required:

    • Customer Focus: Attending to the needs and expectations of customers
    • Group Facilitation: Enabling constructive and productive group interaction
    • Informing: Proactively obtaining and sharing information
    • Oral Communication: Engaging effectively in dialogue
