As a
Housing Social Worker with the Cornerstone Bridge Housing Program, you will provide case management services to the twenty residents living at Cornerstone. Cornerstone will serve adults experiencing chronic homelessness and living with severe and persistent mental illness and/or other disabilities.
Essential Functions:- Provide intensive case management to residents by assessing needs, addressing housing barriers, supporting mental health and substance use and connecting residents to resources
- Develop individualized housing stability plans for each resident and monitor progress towards achieving their goals
- Assist with screening prospective residents and completing initial assessments
- Help clients transition from the bridge housing program to permanent supportive housing
- Facilitate or coordinate groups that are available to Cornerstone Bridge Housing residents
- Orient new residents to the program and assists with addressing incidents as they arise
- Maintains required data and HMIS documentation on all cases
- Assist Housing Support Specialists with weekly unit inspections
- Assist with light housekeeping of units as needed

The Basics (Required Education and Experience) - Bachelor's degree in Social Work or related field
- One year of job-related experience
- Valid Driver's License and "safe" driving record
- Equivalent education and experience are accepted

Beyond the Basics (Preferred Education and Experience) - Master’s degree in Social Work
- Experience working in residential settings
- Two years of experience with case management services, housing and homeless population
How Will We Know You're 'The One'? - Knowledge of Housing First, Harm Reduction, Motivational Interviewing, and other evidence-based practices
- Skills and passion for working with mental health, substance use, crisis intervention and homeless populations
- Ability to prioritize competing responsibilities and accomplish tasks with accuracy and timeliness
- Excellent rapport-building, speaking, listening and written communication skills
- Proficient in Microsoft Windows programs (Excel, Word, Outlook) with the ability to adapt and work collaboratively as a team player
