This recruitment may be used to fill current and future vacancies as they occur in the following program: Sierra Regional Center (SRC). Incumbents are responsible for processing applications for Developmental Services. Duties include conducting Intake Interviews to gather pertinent information to determine eligibility for services including providing information on eligibility criteria and process; promptly following up on individuals' requests for services or assistance; maintaining eligibility data and providing information regarding status of intakes, appeals, transfers and case closures; working collaboratively with Medicaid and service coordinators; and ensuring Person Centered Plans meet all standards of an acceptable PCP including identifying individualized goal(s), Health & Safety Risks, preventative Healthcare and addressing personal rights. Special Requirements Some positions require a valid driver's license at time of appointment and as a condition of continuing employment.
