    The Department of Health Services (DHS) is currently recruiting to fill a Licensed Vocational Nurse position at either the I or II level. If you are interested in applying for the I level of this opportunity, you must apply to the separate concurrent recruitment being conducted at that level.

    NOTE: A current LVN license is required for this position and must be maintained as a condition of employment in this job classification.

    About the Position
    The County of Sonoma’s Department of Health Services is seeking an experienced and compassionate Licensed Vocational Nurse II to join our Youth and Family Services (YFS) outpatient team at the Lakes campus in Santa Rosa. As a LVN II, you will provide essential nursing support to children, youth, and families within our outpatient behavioral health services program. You will have a higher level of independence in your daily tasks, working autonomously with minimal supervision, while still being an integral part of a multidisciplinary care team. This position will work onsite Monday through Friday, 8:30 am - 5:00 pm, excluding County holidays. Responsibilities include:

    • Taking and recording vital signs for clients during on-site appointments
    • Answering client and caregiver inquiries about medications, including side effects, refill issues, and lab work
    • Documenting all services and communications in the electronic health record (EHR) system
    • Communicating urgent needs to psychiatrists and other providers
    • Accepting and documenting verbal medication orders from psychiatrists
    • Managing and administering injectable medications
    • Processing and managing referrals for clients stepping down to lower levels of care
    • Coordinating with primary care providers to request medical referrals, lab work, and follow-up notes
    • Collaborating daily with other LVNs and the wider care team, including prescribers, therapists, social workers, and case managers

    • A strong client-centered and recovery-focused mindset
    • Prior experience working with children, youth, and families (preferred)
    • The ability to thrive in a fast-paced environment and manage multiple priorities
    • Comfort and experience with EHR systems
    • Strong attention to detail, time management, and documentation skills
    • Knowledge of or experience with mental health and/or substance use treatment
    • Familiarity with local community resources and services
    • Bilingual skills (English/Spanish) preferred, but not required

    The Department of Health Services is currently recruiting to fill one full-time Licensed Vocational Nurse position at either the I or II level. This Licensed Vocational Nurse II employment list may also be used to fill future full-time, part-time, or extra help (temporary) positions as they occur during the active status of the list.     Minimum Qualifications

    Experience and Education: Any combination of education and work experience which would provide an opportunity to acquire the knowledge and abilities listed herein. Normally, this would include graduation from and "approved" Vocational Nursing Program and at least six months of recent full-time experience as a Licensed Vocational Nurse in an acute care unit or an inpatient/outpatient program.

    Professional License: Possession of a current California license as a Licensed Vocational Nurse (LVN). LVN license must be maintained as a condition of employment in this classification. Possession of a current CPR and First Aid certification is desirable.

    License: Possession of a valid driver's license at the appropriate level including special endorsements, as required by the State of California, may be required depending upon assignment to perform the essential job functions of the position.

    Knowledge, Skills, and Abilities

    Working knowledge of: vocational nursing theory and practice, rules, regulations and procedures; medical vocabulary commonly used in conveying instructions and orders between doctors, nurses, and licensed psychiatric technicians; mandated client rights, and confidentiality requirements for medical information; California laws governing treatment of involuntary clients; principles of Trauma-Informed and Recovery-Oriented care; medical supplies and equipment, their care and use; basic communicable disease; infection control practices and procedures; basic emergency care; the use, administration, and effects of narcotics and medicines.

    Knowledge of: behavioral patterns and symptoms of clients under the influence of alcohol and substances of abuse.

    Ability to: perform physical assessments; take health histories; assess and review lab results; provide primary nursing care; detect suicidal behavior; follow oral and written directions of nursing technical and professional nature; work as part of a team; communicate with clients empathetically and compassionately; maintain and interpret medical records and reports; write progress notes to Medi-Cal standards; stand and walk for long periods and work under mental and physical strain; occasionally restrain clients; establish and maintain effective working relationships; maintain client rights and privacy as it relates to HIPAA (Health Insurance Portability and Accountability Act of 1996); and abide by established communications codes.
