Job Location: This position will be located at a base location that is flexible within the State of Missouri. This position is telecommuting-eligible if candidate meets all telecommuting eligibilities and qualifications.

- Review and Approve Provider Reassessments
- Lead phone interviews to gather pertinent information
- Utilize a nationally-credited assessment tool
- Review, complete, or authorize care plans and coordinate services to meet participants' unmet needs
- Maintain a queue of assignments in a fast paced environment
- Provide objective and accurate documentation to an interdisciplinary team
- Represent the program with exemplary customer service
- Assume accountability and responsibility over the program's integrity, operations, and success
Minimum Qualifications: - Bachelor's degree or higher level degree from an accredited college or university (relevant experience such as LPN, RN, or social service supports, etc. experience may be substituted for each year of required education) RN & LPN must be licensed and in good standing in Missouri
- Strong interviewing and communication skills
- Keen to detail and documentation
- Exhibit disability & cross-generational awareness
- Prior experience with customer service and working with the elderly, persons with disabilities or other vulnerable population is preferred
