    POSITION

    Under general supervision, manages caseloads of frail elderly, functionally impaired adults, caregivers of older adults, and minor children, high-risk cases suspected of abuse or self-neglect.

    TASKS
    • Manages caseloads of frail elderly, functionally impaired adults, caregivers or older adults and caregivers of minor children, high-risk cases suspected of abuse or self-neglect;
    • Makes home visits and assesses client’s identification, needs, functioning level, and limitations of living independently;
    • Develops, writes, revises, and updates care plans with specific objectives;
    • Arranges services with community agencies;
    • Receives emergency calls and arranges for proper services by outside agencies.
    • Maintains and updates files and progress notes of clients;
    • Attends meetings, conferences, and case presentations;
    • Conducts presentations and educates the community on client needs and programs;
    • Provides information and referrals to caregivers;
    • Recruits caregivers to participate in programs and services;
    • Enforces compliance with contract and grant guidelines and procedures.


    QUALIFICATIONS

    Bachelors degree in Social Work, Public Administration or closely related field, AND two (2) years experience in community education, public relations, or health care service delivery. Must possess at the time of application and maintain a valid California Driver’s License.

    KNOWLEDGE, SKILLS, AND OTHER CHARACTERISTICS
    • Knowledge of contract compliance and grant guidelines and procedures, health assessment techniques and guidelines, and quality file management and record-keeping techniques;
    • Skilled in developing, writing, and maintaining care plans and objectives, providing and following oral and written instructions, and in establishing and maintaining productive working relationships;
    • Ability to perform essential functions with little supervision, to effectively communicate verbally and in writing to clients and management;
    • Proficient in using a personal computer, MS Office Software, and other industry-related programs and software.


    THE SELECTION PROCESS

    Consists of an application review to ensure that all applicants meet the minimum requirements for the job classification. A select number of qualified applicants will be invited to an interview with the Hiring Authority. All employment offers are contingent on a criminal background review via fingerprint submission for applicants over 18 years of age and provision of evidence of freedom from tuberculosis. Every individual the City employs must complete and sign the US Government form I-9 and provide documentation required by law showing that s/he is eligible to work in the United States.
