Public Health Nurse - Care Manager for At Risk Children!
The Care Management for At Risk Children (CMARC) Program, part of the Maternal & Child Health section of the Public Health Division, is dedicated to improving health outcomes and reducing costs for children from birth to 5 years old who meet certain risk criteria. Our multidisciplinary team of Registered Nurses and Social Workers is seeking a passionate Registered Nurse to provide intensive care management services.
Key Responsibilities:
- Provide timely outreach to new referrals following CMARC policies
- Engage members through various platforms including phone, home visits, community, and virtual sessions
- Conduct clinical assessments to identify needs, develop care plans, and assign risk levels
- Document all services in the Electronic Medical Record (EMR) system
- Use motivational interviewing to involve families in decision-making and improve self-care
- Collaborate with medical homes and other providers to ensure cost-efficient, high-quality care

The Basics (Required Education and Experience) - Graduation from an accredited school of Nursing
- No specific experience required
- RN - Registered Nurse - North Carolina Board of Nursing or Compact state
- BPT - Bloodborne Pathogens Training
- CPR - Cardiac Pulmonary Resuscitation
- AED - Automated External Defibrillator
- Health Insurance Portability and Accountability Act (HIPAA)
- Equivalent education and experience are accepted
