    What You'll Be Doing

    Wake County Housing is seeking a compassionate, flexible Registered Nurse-Permanent Housing and Support Services to join the Cornerstone Support Services Team. This team provides specialized support to individuals who have experienced homelessness and are living with chronic health and mental health conditions. The team consists of a disability advocate, a housing navigator, a vocational counselor, a registered nurse and an LCSW supervisor.

    As the Cornerstone Support Services Registered Nurse, you’ll deliver care directly in the community - often in client homes -working closely with case managers and medical providers to improve health outcomes and promote stability.

    This role is ideal for a nurse who enjoys meaningful client relationships, problem-solving in real-world settings and making a difference outside of traditional care environments.

    Key Responsibilities
    • Provide direct nursing care, including assessments, medication administration, and chronic condition management
    • Collaborate with clients, case managers, and healthcare providers to create and implement care plans
    • Educate clients on health conditions and self-care strategies
    • Assist with care coordination, including referrals, appointment scheduling and follow-up support
    • Lead or support health education events and group sessions in the community
    • Deliver services in a range of settings, including client homes, shelters, and the office

    • Graduation from an accredited school of Nursing

    • North Carolina Board of Nursing Licensed Registered Nurse

    Beyond the Basics (Preferred Education and Experience)

    • Master's Degree
    • 3 Year Mental health, psychiatry, working with special needs, chronically homeless populations


    How Will We Know You're 'The One'?

    • Experience working with individuals who are experiencing homelessness and living with severe mental illness, substance use disorders, and complex health issues
    • Ability to develop rapport and trust with clients
    • Knowledge of Harm Reduction and Motivational Interviewing in the context of physical and mental health
    • Ability to work as part of a multidisciplinary team
    • Ability to adapt contents and teaching methods to client’s level of education, cognitive ability and learning style
    • Ability to work independently and maintain professional boundaries
