What You'll Be DoingWe are currently seeking a
Housing - Social Worker to become a member of an interdisciplinary community services team for the
HOPWA (Housing Opportunities for Persons With AIDS) population.
Housing Affordability & Community Revitalization PHSS follows the Housing First Model.
The
HOPWA Housing Social Worker uses a trauma-informed framework to assess client needs and collaboratively develop action plans. Through Motivational Interviewing, Harm Reduction, and other clinical methods, the Social Worker supports clients in achieving health and housing stability. They coordinate access to essential services and resources, with support provided in homes, the field, or agency settings-sometimes requiring on-call availability.
The essential functions of the HOPWA Housing - Social Worker are as follows:- Provide intensive case management, including monthly home visits and personal goal planning
- Understand client health and mental health needs, treatments and their impact on health, housing stability and overall well-being
- Deliver therapeutic services such as crisis support, symptom monitoring and counseling
- Address housing issues with landlords and legal staff to prevent eviction and support rehousing
- Assisting clients with accessing community resources, services and transportation as needed
- Maintain data and HMIS documentation, develop individual service plans for clients and collaborate with internal and external partners to support in care planning and consultation

Not only do more units of affordable housing need to be built, but appropriate services must also be provided to support families and individuals with both accessing and maintaining safe, decent, affordable housing. Affordable housing is also critical to preserving Wake County’s economic competitiveness by offering housing for workers of all income levels, supporting housing stability and economic opportunity for residents and furthering Wake County’s commitment to healthy and inclusive growth. The Department’s programs include: Community Revitalization, Development Financing, Homeless and Prevention Services, Emergency Shelter, Support Services, Veterans Services and Permanent Supportive Housing.
The Basics (Required Education and Experience) - Bachelor's degree in Social Work or related field
- One year of job-related experience
- Valid Driver's License and "safe" driving record
- Equivalent education and experience are accepted

Beyond the Basics (Preferred Education and Experience) - Three (3) years of work experience with case management services, housing, homeless, HIV and special needs populations
- Licenses/Certifications; LCSW - License Clinical Social Worker, CPR Certification - Cardiac Pulmonary Resuscitation (within 60 days) and First Aid Certification (within 60 days)
- Bilingual in Spanish

- Skills in data entry, reporting and basic math for effective data management to produce quality work
- Excellent organizational and administrative skills with keen attention to detail
- Ability to multitask, adapt and collaborate in a fast pace, changing environment
- Excellent written and verbal communication skills with Samaritan House constituents from different backgrounds even in high-pressure settings
