    Statewide Senior MOCDD Support Coordinator

    *Typical working hours: 8:00 AM to 4:30 PM, Monday through Friday.*

    Hybrid Work Schedules/Flexible Schedules/No On-call

    This position can be domiciled at any Department of Mental Health Regional Office State-wide and travel is required for this position.

    Role Description:

    This position will serve as a MOCDDs Support Coordinator supporting families to request the Missouri Children with Developmental Disabilities (MOCDDS) wavier. MOCDDs Senior Support Coordinator Specialist will serve as the subject matter expert and provide ongoing follow up, coaching, and necessary training to other Support Coordinators regarding the MOCDD waiver and the approved steps taken to secure the wavier.

    MOCDDs Support Coordinator duties include assessing, planning, coordinating, and monitoring supports and services for the individual. This will include having regular contact with individuals, including face-to-face visits, linking to community resources and agency programs, making necessary referrals for funding and resources, and implementing an annual plan to guide individual services and supports.

    What you’ll do:

    • Facilitate access to MOCDDS waiver through completion of eligibility and application process - including additional requirements for waiver and MoHealthNet (Medicaid).
    • Able to complete necessary screening tools and assessments to assist individuals in identifying their needs, wants, and goals. (ie: Missouri Values Test , Health Risk Screening Tool , PCSP Assessment, Charting the LifeCourse Tools , etc.)
    • Able to compose Individual Support Plans (PCSPs) which enable the individual to live their good life and achieve their goals.
    • Able to meet PCSP requirements identified in the Individual Support Plan (PCSP) Guide , including maintaining current plans that impel services.
    • Develops positive working relationships with individuals, families, providers, and other team members.
    • Advocates on behalf of the individual and attends meetings when requested (Vocational Rehabilitation, IEP meetings, Children’s Division, etc.)
    • Ability to access or locate available funding programs, including, but not limited to, Community Resources and Grants, Medicaid Waiver Programs, Autism Projects, State Plan Services (Medicaid), County Board Funding, and other Division funding.
    • Creates and calculates budgets/authorizations that align with individual needs in an ongoing manner.
    • Learn and implement defined best practices for submitting PCSP packets and following the Utilization Review process.
    • Follows Division Directive 3.020 Individual Support Plan Monitoring and Review including completing quarterly summaries, monitoring individuals’ services, reviewing provider monthly summaries, and ensuring the PCSP remains accurate and up to date.
    • Contacts individuals in accordance with Division Directive 3.020 , including traveling for in-person meetings with individuals as outlined.
    • Coordinates transitions and administrative transfers for individuals who move out of the coverage area.
    • Demonstrates use of the Targeted Case Management logging system through accurately completing logging requirements as defined in the TCM Technical Assistance Manual .
    • Completes the monthly logging report and meets State SC logging expectations.
    • Maintains an organizational system to ensure completion of all required caseload components by their due dates.
    • Utilizes reports in the Case Management System to plan and prioritize caseload needs.
    • Information on caseload is kept current and at a minimum includes address, phone number, support plan date, consumer resources.
    • Attend team meetings and one on one meetings to discuss caseload progress, staffing, and professional progress.
    • Works to share resources across team and caseload (universalization of knowledge)
    • Completes all necessary training as outlined by the Division within the appropriate timeframe.
    • Utilizes online training resources, conferences, webinars, etc. for continued learning and caseload needs.


    Typical Qualifications:

    • Bachelor's Degree from an accredited college or university, or Registered Nurse license; and 2-4 years' experience which includes but is not limited to, fulfilling the role of a support coordinator, senior support coordinator, or a support coordinator supervisor (or comparable roles within contracted TCM entities).
