The Nutrition Case Management program in the Monterey County Health Department Public Health Bureau, provides support to clients and their families who have weight management, chronic disease, and related dietary concerns. The Nutrition Case Management program in the Monterey County Health Department Public Health Bureau, provides support to clients and their families who have weight management, chronic disease, and related dietary concerns. As Registered Dieticians, the Public Health Nutritionists II assess clients’ nutritional concerns and needs, and work with their clients to set their personal health goals related to nutrition through home visitation and/or telehealth appointments. Services focus on Medi-Cal clients across the age span but prioritize children. Assistance is given on management of pre-diabetes, diabetes, and gestational diabetes management; as well as obesity, dyslipidemia and altered eating patterns related to social, environmental, and behavioral factors.
