Description: JOB DESCRIPTION SUMMARY: The
Navigator Coach is a grant-funded position that providesyou with the opportunity tomakea differencein the lives offamilieswhile youhelpthem navigate resources and achieve short-and long-term goals.The
Navigator Coach worksdirectly with families to create long-term self-sufficiency plans by facilitating small group meetings around topics that include education, employment, economic stability, health, and wellness.The Coachwillmanage anongoing caseloadand will be responsible for identifying , researching, and establishing a connection with resources and servicesthroughoutJefferson Countyand connecting those resources to participantsof Jeffco Prosperity Partners .
SCHEDULE: This position operates primarily in-person on a four- or five day work week, which can be discussed with the hiring manager. Offsite community outreach and mobile work is required . Some flexibility is necessary on an as-needed basis.
Candidates should expect to work some nights, with Thursdays being the usual requirement
COMPENSATION: Hiring Range: $24.62- $26.26 USD Hourly
Compensation will be determined based on education, experience and skills.

ESSENTIAL DUTIES: - Perform in-depth intensive case management to identify needs through intake, enrollment, home visits, and long term support
- Collaborate to create individualized case plans and coordinating resources with community partners to support families in the following areas; fatherhood, education, employment housing, transportation, special needs, wellness, finances, child care , and parenting
- Assist families in becoming advocates for themselves and their children
- Provide one-on-one coaching utilizing a multigenerational approach
- Coordinate, develop, and facilitate topics for JPP small group meetings
- Collaborate with various resources and entities (such as Head Start) throughout Jefferson County
- Present and communicate changes in resource eligibility and enrollment processes and provide resource and strategic referral information to families and members of the JPP team
- Maintain a network of community partners that can be accessed to meet the needs of JPP participants
- Serve as a key member of the JPP Family Advisory Committee and Partners group
- Gather, review, and assess data regarding the local service delivery system and families' experiences when accessing service including gaps in services, duplication of services, access barriers, etc.
- Maintain information for every child and family member in a database
- Other duties and responsibilities as assigned

Minimum Qualifications:
Experience: 3 years of experience in an area such as case management, coaching, counseling, working with diverse and vulnerable populations, crisis coordination, family engagement in community-based social service agencies, schools, and/or non-profit organizations .
Education: Bachelors degree in social sciences , such as social work, psychology, teaching, or related.
Preferred knowledge, skills and abilities :
Industry-specific Knowledge:
- Experience working with diverse, vulnerable populations, including those experiencing poverty
- Life experience which has fostered empathy for our clients
- Experience and knowledge of the two-generation (2Gen) model
- A desire to take a holistic coaching approach and connect with your community
- Experience in the mental health field (counseling, social work, or coaching) or teaching
- Experience with Motivational Interviewing
- An understanding and appreciation for Person-In-Environment theory
- A passion for families and a commitment to the mission of JPP
- Collaborate with various resources and entities (such as Head Start) throughout Jefferson County
- Maintain a network of community partners that can be accessed to meet the needs of JPP participants
