Normal Work Days/Hours: Tuesday-Thursday, 11:00 a.m.-8:00 p.m. and Friday-Saturday, 2:00 p.m.-11:00 p.m.

Responsibilities
The REACHKC program is pre-arrest diversion initiative operating within the Prospect Avenue Corridor, spanning from 27th Street to 45th Street. This program offers low-level offenders an alternative to incarceration, connecting them to community resources and long-term case management instead. Through REACH, the Kansas City Health Department aims to improve public order by creating new systems of care for individuals experiencing mental health challenges, extreme poverty, or substance abuse. REACH Navigators serve as long-term case managers for REACH clients. REACH Navigators work with clients to generate personal goals, will connect clients to community resources, and assist clients in acquiring sufficient services and supports to meet their self-identified goals. Employees serving in this role must be comfortable working with marginalized communities, including but not limited to those suffering from addiction and homelessness. Employees in this role should expect to spend equal time in the office and out in the community, and must be willing to work non-traditional working hours, including on-call shifts, weekends, and evenings.
- Provides long-term case management for REACH clients.
- Meets with clients regularly-both in the office and outside of it-to develop goals for their time in the program.
- Assists clients in meeting their self-determined goals by connecting them with community resources and acting as their advocate.
- Engages with clients, providing referrals, and advocating for their needs.
- Utilizes trauma-informed, racially, and culturally informed practices and harm reduction techniques.
- Offers support in situations involving mental health, substance misuse, homelessness, and more.
- Provides crisis intervention services in partnership with law enforcement and community resource providers.
- Supervises program activities involving cooperation with other community and governmental services, generation of public interest and participation, and coordination of permanent and volunteer staff.
- Supports law enforcement in reducing non-emergency deployments.
- Assists in neighborhood canvassing, personal visits, social service agency outreach, house meetings, and phoning.
- Delivers speeches and assists in writing reports and articles to stimulate public interest.
- Engages with city residents, visitors, and community stakeholders to address challenging client behavior.
- Disseminates information to the community regarding special public health programs.
- Works with confidential information/data needed to carry out responsibilities, adhering to strict program, local, state, and federal confidentiality policies.
- Maintains required records and prepares assigned reports.
- Collects and maintains documentation of all contacts and progress with participants, patients, and visitors.
- Performs other duties as assigned.
Qualifications
REQUIRES an accredited Bachelor's degree OR High School Diploma and four (4) years of experience in private, public, or community healthcare, community sanitation, health inspections, health education, data analysis, or related healthcare fields OR An Accredited Associate's degree and two (2) years of experience. Must pass a physical examination as prescribed by the City. May be required to pass a pre-employment drug screen. Must pass a background check as prescribed by the City.

Positions requiring a high school diploma/GED certificate must be obtained from a school/program accredited by the Department of Education and recognized by the U.S. Secretary of Education.
The minimum education requirement for positions that allow for experience equivalency for an accredited degree is a high school diploma. Unless otherwise specified, an accredited degree must be obtained from a college or university listed with the U.S. Department of Education and recognized by the U.S. Secretary of Education.
For positions with a salary grade of EX6 or higher that require an accredited degree, qualifying professional experience must be obtained AFTER the accredited degree is obtained.
Successful completion of a pre-employment criminal history/background check is required for all positions. Some positions require a post offer drug screen and/or physical.
If appointed, non-residents must obtain residency inside Kansas City, Missouri's city limits within nine months.
